Enrollment Forms
You can either sign up online or print a hard copy of the application form, fill it out by hand, and then fax or mail it back to us.
Doctor's Health Form
Your child's doctor must complete and submit this form to us by June 1. (Even if your child has attended Aloha Beach Camp in the past, we still need a new Doctor's
Form this year.)
Day Camp Code of Conduct
This form must be completed and signed by you and your child and returned to us by June 1. (Even if your child has attended Aloha Beach Camp in the past we still
need a new Code of Conduct Form this year.)
Camp Store Order Form
All campers receive a free t-shirt and rash guard with their enrollment. You can also order additional stuff including 3 t-shirts for the price of 2!, Aloha Beach Camp
baseball caps and new rash guard styles.
Family Handbook
This Handbook answers the most frequently asked questions about camp such as what to wear and bring, general transportation info, camp policies and other
important information. Please review this Handbook with your child so you both know what to expect this summer.
Employment Forms
If you would like to apply for a summer job with us, click here to learn about the various positions we may have available and other important details about
working with us. You will be able to access the online Employment Application from this page.
If you have already completed an Employment Application, we will need a few additional pieces of paperwork to complete your employment inquiry including two
Employment Reference Forms and one Reference Check Release Form.
1. Employment Reference Forms. Please give these forms to two different people who can evaluate you in a professional capacity (NOT friends or family members).
Our fax number and address are printed on the forms, so your evaluators can fax or mail the forms back to us.
2. Reference Check Release Form. This form grants us permission to check your references and to verify the information you have stated on your Employment
Application. Please complete this Form and fax or mail it back to us as soon as possible.
